Longevity Report Card
Upload your de-identified labs and health data. Get a 0–100 score across 19 body systems from the evidence-weighted algorithm Dr. Sorrell built, plus the biggest levers you can pull.
Your report card is one step away
Read the beta terms and tell us where to send health updates. It takes ten seconds.
Add your data
Labs, body-composition scans, fitness tests, sleep data, questionnaires. Add as many files as you like; we combine them.
CSV, Excel, Word, Markdown, text or JSON are read in your browser.
PDFs and photos of lab reports can be read with AI.
- Download the template and open it in Excel, Google Sheets or Numbers.
- Type your results in the value column. Leave anything you don't have blank.
- Add a unit if yours differs (mmol/L is fine) and a date if you know it.
- Save as CSV (or keep it as .xlsx) and upload it on the first tab.
De-identified data only. Remove your name, date of birth, address and record numbers before you upload. Scoring runs in your browser; nothing is saved by Sorrell MD.
Check what we found
Fix anything that looks wrong, add what's missing, then generate your report. Calculated values (like HOMA-IR or eGFR) update automatically.
| Marker | Value | Unit | Date | Remove |
|---|
Your report is calculated in this browser. The written summary is drafted by AI from the scores alone, never your name or email.
How the score works
The Longevity Report Card runs Dr. Sorrell's Systems Health Score (spec 1.0.0): a transparent, evidence-weighted algorithm that turns every data point you have into a score for each body system and one overall number, with an honest read on how complete the picture is.
Every marker gets a score
Each result maps to 0–100 against five anchor points drawn from outcome research. U-shaped markers, like sleep duration, are penalized at both ends.
Markers roll up into systems
Within each of the 19 systems, markers are weighted by evidence: Tier A (hard outcomes, often trial or genetic support) counts most, Tier D least.
Systems roll up into one score
Systems are weighted by their independent link to mortality and healthspan. A geometric blend makes one very weak system pull the total down.
Caps and confidence
Serious findings cap the score so they can't be averaged away. A ± band shows how much more data could move the number.
System weights
Points out of 100 each system contributes when it has enough data. Systems without enough data are left out and their weight is shared among the rest.
| System | Weight | Markers | Why it matters |
|---|---|---|---|
| Cardiovascular | 15 | 38 | Heart disease is still the leading cause of death; ApoB, blood pressure, CAC and Lp(a) each predict events. |
| Cardiorespiratory fitness & exercise physiology | 11 | 17 | VO₂max is the strongest modifiable predictor of all-cause mortality. |
| Metabolic / glycemic | 10 | 21 | Insulin resistance and high blood sugar sit upstream of heart disease, cancer, dementia and fatty liver. |
| Body composition | 5 | 11 | Visceral fat and low muscle mass predict mortality independent of BMI. |
| Musculoskeletal, strength & physical function | 8 | 22 | Grip strength, gait speed, balance and bone density each track survival. |
| Renal | 5 | 10 | eGFR and urine albumin are independent, graded mortality predictors. |
| Hepatic | 4 | 18 | Liver fibrosis and GGT predict liver and all-cause mortality. |
| Hematologic | 2 | 16 | Anemia, RDW and white-cell count predict mortality, partly downstream of other systems. |
| Immune / inflammatory | 4 | 13 | hs-CRP, IL-6 and fibrinogen capture inflammation that drives vascular disease. |
| Endocrine | 4 | 30 | Overt thyroid, adrenal and gonadal dysfunction raise mortality; optimization ranges are weaker evidence. |
| Neurological, cognitive & sensory | 6 | 21 | Cognition, brain imaging, hearing and smell predict dementia and survival. |
| Sleep & circadian | 5 | 18 | Sleep apnea burden, duration and regularity all track mortality. |
| Respiratory | 4 | 12 | FEV1 predicts mortality even in people who never smoked. |
| Gastrointestinal & microbiome | 2 | 14 | Few strong markers outside cancer screening; microbiome data is still early. |
| Preventive care & cancer surveillance | 5 | 17 | Up-to-date cancer screening and vaccines carry trial-level mortality benefit. |
| Nutritional / micronutrient | 2 | 18 | Vitamin D, omega-3 index and B12 matter, with mixed supplement trials. |
| Mental health & psychosocial | 4 | 14 | Depression and social isolation raise mortality roughly 1.3–1.5×. |
| Lifestyle & behavioral exposures | 3 | 15 | Scores the exposures themselves (smoking, alcohol, activity, diet); their effects are scored elsewhere. |
| Environmental / toxicology | 1 | 10 | Lead, cadmium and air pollution matter at population scale; small individual weight. |
Weights are expert-set from published effect sizes (spec §2). They will be recalibrated against national mortality data before the beta label comes off.
What the numbers mean
Every marker it scores
Use these names, or the IDs in the template, in your file. Helper values like creatinine, cystatin C, total cholesterol, height and weight are used to calculate eGFR, BMI and other scored markers.

Questions
What data can I upload?
The easiest and most accurate option is our CSV template: open it in Excel, Google Sheets or Numbers, fill in the values you have and upload it. The tool also reads CSV or Excel exports, Markdown, plain text you paste from a lab portal, Word documents and JSON. PDFs and photos of lab reports can be read with AI. Remove your name, date of birth, address and record numbers first.
Is my data stored?
No. Scoring runs entirely in your browser and Sorrell MD does not save your results. If you ask the AI to read a document, and when the report summary is written, the de-identified data is sent securely to our AI provider (Anthropic) to process; Sorrell MD does not store it. Your name and email are kept separately and are never linked to your health data.
How is the score calculated?
Each result is scored 0 to 100 against evidence-based anchor points. Results roll up into 19 body systems, weighted by the strength of their outcome evidence (tiers A to D). Systems roll up into one Overall Health Score, weighted by each system's contribution to mortality and healthspan, with a blend that penalizes one very weak system. Safety caps keep a serious finding from being averaged away. The same inputs always give the same score.
What do completeness and the ± band mean?
Completeness is how much of the algorithm's evidence-weighted panel your data covers. A system needs at least 35% coverage to be scored. The ± band widens as completeness falls: with half the data, the band is about ±7.5 points. More data gives a sharper score, not automatically a higher one.
Why is my score capped?
Some findings limit the score no matter how good everything else is: current smoking, a coronary calcium score of 400 or more, eGFR under 30, untreated severe sleep apnea, HbA1c of 8% or more, and a few others. The report names every cap that applies so you can see exactly what is holding the number down.
How was this built, and where does AI come in?
Dr. Sorrell designed the Systems Health Score from published outcome research, using AI tools to help build and test it. In the tool, AI (Anthropic's Claude) can read documents you upload and writes the plain-English summary of your report. The score itself comes from a fixed, transparent formula, not from AI. AI can make mistakes, so check every value it reads.
Why is it called a beta?
The weights are expert-set from published effect sizes. Calibration against national mortality data (NHANES) is still in progress, and the formula will change as it is validated. Scores from different versions are not directly comparable; each report shows its spec version.
Is this medical advice?
No. The Longevity Report Card is an educational tool. It does not diagnose, treat or prevent any condition, it does not create a doctor–patient relationship, and it is not a substitute for a clinician who knows your history. Talk with your doctor before acting on anything in it. In an emergency, call 911.
The Longevity Report Card is a beta, educational tool built with the help of AI. It is not medical advice, a diagnosis or treatment, and it does not create a doctor–patient relationship.
