“My Cholesterol Is Fine” Is Not the Same as Low Risk
The standard lipid panel measures total cholesterol, LDL, HDL and triglycerides. It is genuinely useful and I order it constantly. It also misses one of the strongest inherited drivers of cardiovascular risk we know of — not because the result is subtle, but because the particle simply is not on the test.
Lipoprotein(a) — written Lp(a), said “L-P-little-a” — is an LDL particle with an extra protein wrapped around it. That addition makes it more likely to lodge in artery walls, more inflammatory once it does, and more prone to promoting clot formation. It also accelerates calcification of the aortic valve, which is why it turns up in aortic stenosis as well as in coronary disease.
Two features make it unlike the rest of your lipid panel. First, your level is around 90% genetically determined and essentially fixed for life — it does not drift with diet, exercise or weight the way LDL does. Second, statins do not lower it; in some people they nudge it slightly upward even while doing their real job on LDL. So this is not a risk factor you can manage by doing the usual things more diligently.
Roughly one in five people worldwide carry a level high enough to matter clinically, and the European Atherosclerosis Society recommends measuring it at least once in every adult. Most people reading this never have. If you have a family history of early heart disease alongside unremarkable cholesterol numbers, this is very often the missing piece of that story.

Above roughly 125 nmol/L, your baseline risk sits higher than the rest of your numbers imply. That is not a sentence and it is not an emergency — it means everything modifiable around it gets tighter targets: LDL and ApoB driven lower, blood pressure controlled harder, imaging considered earlier.
Practical read: the same tools, used more aggressively.
It Runs In Families
Because the level is inherited, your result is also information about your parents, siblings and children — each of whom has meaningful odds of carrying it too. In practice, one person's elevated result is frequently the reason an entire family finally gets screened, sometimes catching a sibling decades before anything would have surfaced.
Targeted Therapy Is Close
For decades there was no drug that specifically lowered Lp(a), which is a large part of why testing never became routine — clinicians are reluctant to measure what they cannot treat. Several agents designed to do exactly that are now in late-stage trials. Knowing your number now means you are already identified if and when that landscape changes.

Ask for it by name
“Lipoprotein(a)” — and where possible ask for the result in nmol/L rather than mg/dL, since the mass-based unit is less consistent across laboratories. It is a routine, widely available, inexpensive test. It just has to be ordered specifically, because it is never included by default.

Add ApoB to the same draw
ApoB counts every atherogenic particle in circulation rather than estimating the cholesterol inside them, and it reads risk better than LDL-C alone — particularly in anyone with metabolic syndrome or high triglycerides. Two additions to one blood draw tell you far more than the standard panel does.

Write your family history down properly
Any first-degree relative with a heart attack, stent, bypass or stroke before 55 in men or 65 in women. That single fact meaningfully changes how everything else should be interpreted, and most people have never been asked for it in a way that made them go and check.
